Sustaining the fight against COVID-19,Indian Railways is making an all out effort to supplement the health care efforts of Government of India. The spectrum of steps include equipping the existing Railway Hospitals to meet the needs for COVID-19, earmarking of hospital beds to meet the contingencies, recruitment of additional doctors & paramedics, modification of passenger coaches as isolation coaches, availability of medical equipments, in-house production of PPEs and Ventilators, etc.
Health Department of Railways is getting ready and equipped to supplement the healthcare efforts of Government of India in its fight against COVID-19 .It may be noted that Railways have  a chain of 586 Health Units, 45 Sub Divisional Hospitals, 56 Divisional Hospitals, 8 Production Units Hospitals and 16 Zonal Hospitals spread all over the country. A significant portion of these facilities will now be dedicated to combat the Corona virus.
With a total of 2546 posts of Doctors and 35153 paramedic staff including Nursing staff, Pharmacist and other category of staff, Railways is ready to face any challenge in the fight against COVID-19. In a new initiative Railway Health Services have been made available to all Central Government Employees across the country now. The services include Primary, Secondary, and tertiary care in certain Specialties. 
Following initiatives have been taken by Indian Railways in the fight against COVID-19:-
            1. Conversion of coaches to serve as quarantine/isolation facilities: Indian Railways is converting 5000 train coaches across the country to serve as quarantine/isolation facilities for 80000 beds for COVID-19.Action has been started by Zonal Railways on a war footing. Conversion of 3250 coaches has already been completed.
            2. 5000 beds earmarked for treatment of COVID patients: Around 5,000 beds in 17 dedicated hospitals and 33 hospital blocks in Railway Hospitals have been identified for treatment of COVID-19 patients. These hospitals and blocks are being made ready for this purpose.
            3. 11,000 quarantine beds: 11,000 quarantine beds have been made available across the Indian Railways establishments to fight COVID-19.
            4. Availability of medical equipment-Ventilators and PPEs: Availability of sufficient number of Personal Protective Equipment (PPE), Ventilators etc. would be critical in fight against COVID-19. Action has been taken by Railway zones and production units to procure ventilators, PPEs and medical equipment to meet their needs to fight COVID-19.
            5.It may be noted that Railways has already started  In-house  production of  Personal Protective Equipment (PPE). It is striving to produce around 1000 PPEs per day which may be upscaled further.
            6.Railway Health Services offered to all Central Government Employees: Railway Health Services have been made available to all Central Government Employees across the country on showing their Identity cards at the Railway Hospitals/Health Centres.

As corona virus wrecks havoc in many countries around the world and thousands of people have lost their lives, Indiahas stayed relatively unscathed. To counter the emerging threat of corona virus,Northern Railway Central Hospital gears up to tackle with corona virus.One dedicated ward for clinically suspected cases has been established at Northern Railway Central Hospital New Delhi with personnel protective disposable dress for both paramedics and patients.
Lecture o­n Do’s and Don’ts by health educators alongwith Railway Doctors at Railway Colonies, Club, Health Units and Barat Ghars gathering shall be done. Peoples will be educated about the symptoms of the corona virus and if any o­ne has the symptoms, he may immediately contact to nearest Health Unit or Hospital. Ayushman Bharat –Pradhan Mantri Jan AarogyaYojana (AB-PMJAY) to be launched by Prime Minister Shri Narendra Modi in Ranchi, Jharkahnd on September 23, 2018 
India takes a giant leap towards providing accessible and affordable healthcare to the common man with the launch of Ayushman Bharat – Pradhan Mantri Jan AarogyaYojana (AB-PMJAY) by the Prime Minister, Shri Narendra Modi on 23rd September, 2018 at Ranchi, Jharkhand. Under the vision of Ayushman Bharat, Pradhan Mantri Jan AarogyaYojana (AB-PMJAY) shall be implemented so that each and every citizen receives his due share of health care. With Ayushman Bharat – Pradhan Mantri Jan AarogyaYojana, the government is taking healthcare protection to a new aspirational level. This is the “world’s largest government funded healthcare program” targeting more than 50 crore beneficiaries. 
BENEFITS UNDER THE SCHEME: 
Ayushman Bharat- Pradhan Mantri Jan ArogyaYojana (PMJAY) will provide a cover of up to Rs. 5 lakhs per family per year, for secondary and tertiary care hospitalization. 
  • Over 10.74 crore vulnerable entitled families (approximately 50 crore beneficiaries) will be eligible for these benefits. 
  • PMJAY will provide cashless and paperless access to services for the beneficiary at the point of service. 
  • PMJAY will help reduce catastrophic expenditure for hospitalizations, which impoverishes people and will help mitigate the financial risk arising out of catastrophic health episodes. 
  • Entitled families will be able to use the quality health services they need without facing financial hardships. 
  • When fully implemented, PMJAY will become the world’s largest fully government-financed health protection scheme. It is a visionary step towards advancing the agenda of Universal Health Coverage (UHC). 
FEATURES OF THE SCHEME 
Ayushman Bharat is a progression towards promotive, preventive, curative, palliative and rehabilitative aspects of Universal Healthcare through access of Health and Wellness Centers (HWCs) at the primary level and provision of financial protection for accessing curative care at the secondary and tertiary levels through engagement with both public and private sector. 
It adopts a continuum of care approach, comprising of two inter-related components: Creation of 1,50,000 Health and Wellness Centres which will bring health care closer to the homes of the people. 
These centres will provide Comprehensive Primary Health Care (CPHC), covering both maternal and child health services and non-communicable diseases, including free essential drugs and diagnostic services. The first Health and Wellness Centre was launched by the Prime Minister at Jangla, Bijapur, Chhatisgarh on 14th April 2018. 
The second component is the Pradhan Mantri Jan ArogyaYojana (PMJAY) which provides health protection cover to poor and vulnerable families for secondary and tertiary care. 
The Health and Wellness Centres will play a critical role in creating awareness about PMJAY, screening for non-communicable diseases, follow-up of hospitalization cases among others. The features of the scheme are as follows. 
Pradhan Mantri Jan ArogyaYojana: Financial protection from catastrophic expenditure:
  • 71st Round of National Sample Survey Organization (NSSO) has found 85.9% of rural households and 82% of urban households have no access to healthcare insurance/assurance. More than 17% of Indian population spend at least 10% of household budgets for health services. Catastrophic healthcare related expenditure pushes families into debt, with more than 24% households in rural India and 18% population in urban area have met their healthcare expenses through some sort of borrowings. 
  • PMJAY primarily targets the poor, deprived rural families and identified occupational category of urban workers’ families as per the latest Socio-Economic Caste Census (SECC) data for both rural and urban areas as well as the active families under the RashtriyaSwasthyaBimaYojana (RSBY). 
  • Approximately 10.74 crore identified families (approximately 50 crore beneficiaries) will be entitled to get the benefits. There is no cap on family size and age as well as restriction on pre-existing conditions.
Pradhan Mantri Jan ArogyaYojana: Hospitalization cover from inpatient care to post hospitalisation care: 
  • The objectives of the Yojana are to reduce out of pocket hospitalisation expenses, fulfil unmet needs and improve access of identified families to quality inpatient care and day care surgeries. 
  • The Yojana will provide a coverage up to Rs. 5,00,000 per family per year, for secondary and tertiary care hospitalization through a network of Empanelled Health Care Providers (EHCP). 
  • The EHCP network will provide cashless and paperless access to services for the beneficiaries at the both public and private hospitals. 
  • The services will include 1350 procedures covering pre and post hospitalization, diagnostics, medicines etc. 
  • The Yojana beneficiaries will be able to move across borders and access services across the country through the provider network seamlessly. 
Pradhan Mantri Jan ArogyaYojana in alliance with the States: 
  • The scheme architecture and formulation has undergone a truly federal process, with stakeholder inputs taken from all States and UTs through the national conclaves, sectoral working groups, intensive field exercises and piloting of key modules. 
  • The Scheme is principle based rather than rule based, allowing States enough flexibility in terms of packages, procedures, scheme design, entitlements as well as other guidelines while ensuring that key benefits of portability and fraud detection are ensured at a national level. 
  • States have the option to use an existing Trust/Society or set up a new Trust/Society to implement the Scheme as State Health Agency and will be free to choose the modalities for implementation. 
  • States can implement the Scheme through an insurance company or directly through the Trust/Society/Implementation Support Agency or a mixed approach. 
PROGRESS OF THE SCHEME:
MOUs with the states: 30 states and UTs have signed the MoU and have started working on implementation of the mission. Maharashtra has joined on 14th August 2018. MoU has been signed with Tamil Nadu on 11th September 2018. 
Pilot launch of PMJAY: 
Pilot launch of the scheme has started. The focus of the pilot launch of the mission is to test and enhance the developed IT systems and streamline the state preparedness for a comprehensive launch. 
The pilot been done in around 22 States / UTs (Arunachal Pradesh, Chandigarh, Chhattisgarh, Dadar Nagar & Haveli, Daman & Diu, Haryana, Himachal Pradesh, Madhya Pradesh, Manipur, Meghalaya, Mizoram, Nagaland, Uttarakhand, West Bengal, Uttar Pradesh, Jammu & Kashmir, Goa, Maharashtra among others). 
The pilot launch is taking place in specific hospitals to test the Beneficiary Identification System (BIS) and Transaction Management System (TMS) systems.
 The pilot launch involves over 1280 hospitals. 
Pradhan MantriAarogyaMitra (PMAM): 
  • The scheme is creating a cadre of certified frontline health service professionals called Pradhan MantriAarogyaMitras (PMAMs) who will be primary point of facilitation for the beneficiaries to avail treatment at the hospital and thus, act as a support system to streamline health service delivery. 
  • MoU was signed between National Health Agency and Ministry of Skills Development & Entrepreneurship (MSDE) on 27th August 2018 in New Delhi. 
  • AarogyaMitras training is being conducted in collaboration with National Skill Development Corporation (NSDC) and Ministry of Skill Development to strengthen implementation and operational preparedness. 
  • Training has already been conducted in across 20 states and around 3519 AarogyaMitras have been trained so far. Training programs for State, District and PMAMs have been conducted in 27 states. 
  • A total of around 3936 personnel have been trained at state, district and PMAM levels.
Hospital Empanelment: 
So far 15,686 applications for hospital empanelment have been received. 
 Fraud detection and Data privacy: 
  • Detailed guidelines have been prepared to address the issues around potential fraudulent activities that could be committed by any individual or organization. 
  • Anti-fraud cells will be established at the national and state level, and strong IT tools will be deployed to prevent and detect fraud. 
  • NHA Information Security Policy & Data Privacy Policy are being institutionalized to provide adequate guidance and set of controls on the secure handling of Beneficiaries Personal Data & Sensitive Personal Data in compliance with all laws and regulations applicable. International standards/regulation, like ISO 27001, National Institute of Standards & Technology (NIST), Health Insurance Portability and Accountability Act (HIPAA), General Data Protection Regulation (GDPR); and Indian regulations such as Aadhaar Act 2016, Indian IT Act 2000, IT (amendment) Act, 2008 and Personal Data Protection Bill by former judge BN Sri Krishnan (Yet to be enacted) are referenced while drafting these policies. 
IT systems update:                                                                    
  • Beneficiary Identification System (BIS) has been developed to identify and verify the beneficiaries at CSCs and point of care. 
  • NHA has tested the software in 80 districts across 10 states and further enhanced the system based on feedback. 
  • Hospital empanelment module has been active from 4th July and has already received empanelment requests from over 7,857 hospitals across 354 districts in 22 States / UTs. A comprehensive 
  • Transaction Management System (TMS) has been developed to facilitate transactions from hospitals (such as filing pre-authorization requests and claims submission). 
  • There have been trainings across the state concerning the operability of IT systems to enhance the state preparedness and receive feedback on the IT systems. 
  • Focused workshops have been organized to train the state level officials in operating the developed IT systems. 
  • Robust measures are being put in place in the IT platform to ensure privacy and security of the data. 
Beneficiary related: 
  • MoU has been signed with Common Service Centre (CSC) for Beneficiary Identification and will utilize the over 3 lakh village level entrepreneurs for identifying beneficiaries. 
  • Personalized Letters with unique family code are in the process of being sent to the identified families in the Additional Data Collection Data (ADCD) drive. This will drive awareness among the beneficiaries and further ease the identification process when they visit points of care or CSC centres. 
Ayushman Bharat – Pradhan Mantri Jan AarogyaYojana (AB-PMJAY) is a paradigm shift from sectorial, segmented and fragmented approach of service delivery through various national and State schemes to a bigger, more comprehensive and better converged and need based service delivery of secondary and tertiary care.

Subject: Revision of rates of subscription under Central Government Health Scheme due to revision of pay and allowances of Central Government employees and revision of pension/ family pension on account of implementation of recommendation of the Seventh Central Pay Commission.
The undersigned is directed to refer to this Ministry’s ‘OM No. S.11011/11/2016-CGHS(P) dated 20th Ma’y, 2009 vide which orders were issued revising the rates of monthly subscription for availing CGHS facility, as also the entitlement for free diet, entitlement of accommodation in private empanelled hospitals under CGHS, etc.
2. Consequent upon revision of pay on the basis of the implementation of the recommendations of the 7th Central Pay Commission, it has been decided to revise the rates of subscriptions, to be made by employees/ pensioners, for availing benefits under the CGHS, with effect from 1st January 2017. It has also been decided to revise the monetary ceiling limits for various entitlements of the beneficiaries for availing CGHS facilities.
3. In supersession of all earlier instructions, the following revisions are being made, is so far as it relates to the facilities mentioned below:

Dear Sir,

Sub: Abrupt enhancement in per-capita charges for treatment of staff and their family members employed in Railway Cooperative Credit Societies/Banks and other quasi-administrativeboffices staff for treatment in Railway Hospitals

Railway Board vide letter No.2003/H-1/2/2 dated 27.05.2016 have issued instructions to revise per-capita charges for medical attendance and treatment facilities to the staff and the members of their families employed in Railway Co-operative Credit Societies and Banks and other quasi-administrative offices of Indian Railways in railway hospitals w.e.f. 2004-05, i.e. after 11 years.

Per-capita charges in connection with the above are deposited every year by the concerned Railway Co-operative Credit Societies and Banks and other quasi-administrative offices for the staff and their family members for their medical attendance and treatment in Railway Hospitals. At this distant stage, abrupt upward revision of per-capita charges, effective from 2004-05, is not at all justified and would create lot of problems to Railway Co-operative Credit Societies and Banks and other quasi-administrative offices as this expenditure is to be reflected in the balance sheet of the concerned year, which is mandatorily submitted to the concerned Registrar in the stipulated period of time.

AIRF, therefore, demands that this revision may be withdrawn and the issue be discussed with the federation before finalizing the same.

Facilities of Medical Aid in Trains

All passenger carrying trains are provided with the First Aid Boxes containing essential drugs and dressing materials. In addition, Augmented First Aid Boxes with wide range of medicines, disposable medical material, etc. have been provided with the Train Superintendents/ Guards of Rajdhani/Shatabdi Express trains and some nominated trains. Front line staffs deployed on trains are also trained in rendering First Aid. Trains can also make unscheduled halts, if necessary, at the stations en-route in emergencies. The Station Masters of all stations have details of doctors, clinics & hospitals, both Government and Private, in the vicinity of the station, so that their services could also be availed, in emergencies. Ambulance service of both Railway hospitals and State Governments are utilized when required.
A three digit number “138” has been allotted for “Railway help line other than Security”. Passengers can dial this number from their mobile phones during emergencies and contact the Divisional Control for the help/assistance required.

Digital India Initiative - Aadhaar Based Online Registration and Appointment System.

Online Registration System (ORS) is a framework to link various hospitals across the country for Aadhaar based online registration and appointment system, where counter based OPD registration and appointment system through Hospital Management Information System (HMIS) has been digitalized. The application has been hosted on the cloud services of NIC. Portal facilitates online appointments with various departments of different Hospitals using eKYC data of Aadhaar number, if patient's mobile number is registered with UIDAI. And in case mobile number is not registered with UIDAI it uses patient's name. New Patient will get appointment as well as Unique Health Identification (UHID) number. If Aadhaar number is already linked with UHID number, then appointment number will be given and UHID will remain same.

Simple appointment process
For your first visit to hospital, registration and appointment with doctor is made simpler. All you have to do is verify yourself using Aadhaar Number, Select Hospital and Department, Select date of Appointment and receive SMS for Appointment.

Hospital Patient Care Allowance/Patient Care Allowance was introduced on the Railways in terms of Railway Board’s letter no. E(P&A)Il-98/HW-6 dt. 09.01.2008. As per paragraph 2 (a) (ii), of the letter dt. 9-1-2008 Pharmacists were also made eligible for grant of 1-lPCA/PCA subject to fulfilment of the conditions of admissibility except exclusive Store Pharmacists who were not involved in dispensing of medicines. Both the recognised Federations, namely AIRF and NFIR have raised the issue in the forum of PNM stating that there is no particular designation of Store Pharmacists in the Indian Railways. The matter has been considered in consultation with the Health Directorate of Railway Board and it has been decided to remove the exception made for exclusive Store Pharmacists in paragraph 2(a) (ii) in Railway Board’s letter no. E(P&A)ll-98/HW-6 dt. 09.01.2008. Phannacists will be entitled for payment of Hospital Patient Care Allowance I Patient Care Allowance. This would have effect from 01.01.2008 as mentioned in Railway Board’s letter no. E(P&A)li -98/HW-6 dt. 09.01.2008.
3. Other tenns and conditions would remain the same as per Board’s letter no. E(P&A)II – 98/HW-6 dt. 09.01.2008 and Board’s letter No. E(P&A)II-2013/AL-3 dt. 20.02.20 13.
4. This issues with the concurrence of Finance Directorate of the Ministry of Railways.

Eastern Railway has introduced a unique system of registering the names of patients through o­nline system. This will help to minimize the waiting period of the patients and make the process of registration for taking appointment from the super-specialty and multi-specialty hospitals of Eastern Railway hassle free. This has been introduced as per the Hon’ble Prime Minister’s flagship programme “Digital India” scheme.  

In this system, any Railway beneficiary from any corner of India can easily register his/her name by using their AADHAAR Card number for getting appointment for consultation  at B. R. Singh Hospital, Sealdah and Howrah Orthopaedic Hospital of Eastern Railway.

At present, Eastern Railway is the only Railway Zone to introduce such a patient-friendly system under Digital India initiative. The system will be useful for all patients especially for senior citizens and people staying in far-away places. 

This o­nline patient registration system was inaugurated by the Director General, Railway Health Services, Railway Board, New Delhi, Dr. Anil Kumar o­n 24thJanuary, 2016 at Kolkata.

General Manager Inspects Central Hospital, Lallaguda, Secunderabad

Shri Ravindra Gupta, General Manager, South Central Railway inspected Central Hospital, Lallaguda, Secunderabad today i.e. 14th November, 2015. His inspection, in tune with the stress being accorded towards employee welfare centric objective, covered the entire hospital premises including the Intensive Care Unit, Neo-natal Intensive Care Unit, various wards including Obstetric & Gynaecology ward, Physiotherapy Unit, pharmacy stores, Out Patient block etc. 

The General Manager interacted with Dr. Girija Shankar, Chief Medical Director, Central Hospital and other Doctors and sought details on the working of the hospital and the patient demands handled. He advised that the hospital facilities and services shall be enhanced still more, so as to be o­n par with the best. This will ensure that the health and welfare of the railway employees and their dependants are taken due care of.

Trains with Emergency Medical Facilities

All passenger carrying trains are provided with First Aid Boxes containing essential drugs and dressing materials. In addition, Augmented First Aid Boxes with wide range of medicines, disposable medical material, etc. have been provided with the Train Superintendents of Rajdhani/Shatabdi Express trains and Guards of other nominated trains. The front line staff deployed on trains is trained in rendering First Aid. Trains can also make unscheduled halts, if necessary, at the stations en-route in emergencies. The Station Masters have details of doctors, clinics & hospitals, both Government and Private, in the vicinity of the station, so that their services could also be availed, in emergencies.

However, in case of medical emergency passenger can contact helpline number 138 through their mobile or inform on-board Ticket Checker for arranging medical assistance.
